Toxicology UK undertakes its analytical testing at NMS Labs in Philadelphia, an internationally recognised forensic laboratory capable of detecting many hundreds of substances, including prescription medicines, drugs of abuse, alcohol and many Novel Psychoactive Substances. Continuous internal quality controls, external proficiency testing and internationally recognised laboratory ISO standards ensure that every analytical result is accurate, traceable and legally defensible.
Toxicology UK's team of experienced forensic toxicologists then interprets those findings alongside the deceased's medical records, prescription history, known drug use, pathology findings and the circumstances surrounding the death. Contextual assessment transforms analytical data into scientifically defensible conclusions that directly support the Coroner's investigation.
Advances in logistics and tracking have made this model practical. Samples can now be transported safely and securely around the world while maintaining their integrity.

Originally developed by Dr Elliott while working with the European Union to assess designer drugs, the framework provides a structured and consistent way of evaluating each detected substance. It helps communicate the likely contribution of detected substances within the overall circumstances of the case. Factors such as the drug’s pharmacological properties, the concentration detected, the deceased's medical and prescription history and potential interactions between multiple substances are considered.

The results are then presented using a low, medium or high significance rating, supported by the detailed scientific interpretation. Coroners and pathologists can quickly identify the substances most relevant to determining the cause of death while making it easier for families to understand the findings without specialist medical knowledge.
Since introducing it, Toxicology UK has received positive feedback from Coroners, pathologists and families. Dr Elliott also recognises the potential to make the framework available to other forensic organisations through licensing agreements internationally.
Looking Beyond Initial Assumptions
Clear interpretation is only possible when the underlying investigation is equally comprehensive.
The early stages of a death investigation are often guided by incomplete information, based on police enquiries and information gathered from family and friends about possible drug use. As the investigation progresses, new information regarding undisclosed medications, recreational drug use or digital evidence indicating an interest in a particular substance may emerge.
When a substance that was not initially suspected becomes relevant later in the investigation, investigators need to know whether it has already been tested for and excluded. Toxicology UK therefore recommends broad toxicological screening at the beginning of every investigation. Testing for a wide range of substances without preconceived expectations helps investigators confirm what was present and what can be excluded. This minimises the risk of bias, reduces the need for repeat testing when new evidence comes to light and gives Coroners greater confidence that the investigation has considered every reasonable possibility.
While broad toxicology screening provides a strong starting point, it cannot answer every question. Some substances have unique chemical properties that require targeted analytical methods and will not appear in a routine screening. Based on many years of experience, knowing when to pursue additional lines of enquiry distinguishes Toxicology UK's investigative approach.
In one investigation, an individual was found dead at home with no obvious injuries or underlying medical condition that could explain the death. A comprehensive toxicology screen identified only prescribed medications and a small amount of alcohol, both at levels consistent with normal therapeutic or social use. Conventional testing had effectively ruled out the most common causes, yet it failed to find the cause of death.
The absence of a clear toxicological explanation prompted Toxicology UK's scientists to revisit the wider circumstances of the case. During discussions with investigators, they learned that a bowl of custard had been found beside the deceased, which prompted a different line of enquiry. They recommended targeted testing for an industrial chemical commonly used as a food preservative that has increasingly been associated with fatal poisonings and can be taken by mixing in with food. Because the substance requires specific analysis, it would not appear in a general toxicology screen. Subsequent testing confirmed its presence at fatal concentrations, ultimately establishing the cause of death and providing the Coroner and the family with the clarity they had been seeking.
